There are probably 2 types:
Those that do it for the RMT money, which is scummy but hey money is money. They could be doing worse things I guess.
The second type would be those that do it for "fun", they are most likely deeply insecure and can't handle losing in any way so the only way for them to have fun is to guarantee "victory".

When I was young I would hang out in a computer club. This was before broadband was a thing in all homes. So we would play LAN and online. Yes, I am old.
Anyway, without fail, the kids who hacked at CS 1.7 were all either really fat or had personal issues, or both. Although I am not sure about their penis size, knowing this I mostly feel sorry for the kids using hacks.
Edit: Also, this is many years ago of course, and looking at those guys now, I realize their lives did not turn out well for any of them. They're what politicians used to call "social losers" and one of them is even dead.
So, if you're a kid now and feel you need to cheat in games, you gotta get your shit together.

You cant help but feel bad. We all have lives and wins in real life. Jobs, relationships, working out or other hobbies that give us that win in real life. Tarkov is a fun avenue to play and get fun wins. A game. The cheaters are hopeless losers. No wins in real life. Shitty or no job, no relationships worth a damn, no wins in life. They go to tarkov for wins, but they suck. They are low IQ and cant adapt. So they cheat to get the only wins they can manage in their poor pitiful lives. I feel bad for them.

The only 100% sure way you can know is if you play with a group, and all of you get head-eyes’d by the same person within half a second, despite being staggered in such a way that nobody could flick to every head in that amount of time.
Indicators that someone MIGHT be hacking include:
running directly to your position, despite you having remained 100% silent for an extended period of time (could be a spot they always check) zipping along at unnatural speeds (could be desync) they have altyn/faceshield with no body armor (could be meming) they have best-in-caliber ammo despite having a gun with no attachments, sometimes even no ironsights (could be challenging themselves with stock gun / meming) they hit you in your faceshield 5+ times in the span of a second or two (could be really good / have a no-recoil gun like vector) they shoot you through a wall or other hard cover (could have great hearing / game sense) they instantly headshot you when swinging a corner, even though they were only on your screen for a quarter of a second (could be good / desync) -
